Alternatively a hard copy of the form can be collected from our reception desk.
You will be allocated a GP but you will actually be registered with the Practice, therefore will be able to see any available doctor of your choice.
On registration, you will be asked to complete a health questionnaire and you will be encouraged to have a well person appointment with the practice nurse.
Prevention of disease, or injury is a primary concern and the medical staff will advise and inform you of the steps you may take to promote good health and a healthy lifestyle.
